The WH-347 fillable form — two honest options

Option 1 — the official blank form (free)

This is the Department of Labor's current WH-347, straight from dol.gov. The PDF is not a true fillable form — the government distributes it flattened, so most contractors print it and complete it by hand or rebuild it in a spreadsheet.

Why not just a spreadsheet template?

Spreadsheets drift: a deleted row breaks a total, last project's contract number survives into this one, and the compliance statement gets paraphrased until it no longer matches 29 CFR 3.3.
